CMSED Target Population Enrollment
- For clients under the age of 18 with a mental health diagnosis, enrollment or re-enrollment in the CMSED Target Population (At Risk) is done through the approval of the SEC Clinical Review Team.
- The client must be open/active in the Southeastern Center system and meet the CMSED criteria. If you wish to have your client considered for enrollment, complete the enrollment/re-enrollment application form and fax it to Judy Wall at 910-332-6740. Clients who are determined to meet the criteria for CMSED are enrolled for one year (unless that client turns 18 years old during that year) and must be presented for re-enrollment prior to the lapse date on their new CMSED IPRS form. The same criteria must be met for re-enrollment that is required for the initial enrollment. The parent/guardian signature is needed at the time of the original enrollment and for any subsequent re-enrollment.

- It has come to our attention that some clients are still active to someone else. Although we ask the clients directly during the screening, please be sure to ask your client whether or not he/she is active to another provider. A client can only have one "clinical home". Providers will not be reimbursed for services if the consumer is open to another provider.

Diagnostic Assessment
Initial authorizations for state-funded Community Support services for new referrals from Screening, Triage and Referral are limited to ten hours to be used within the first thirty days. These hours are to arrange for the Diagnostic Assessment (5006) or MH Evaluation (3300) and develop the Person-Centered Plan. For additional information on pre-authorized services, please see the memo dated 1/25/07 "Changes to the IPRS Authorization Process" posted on the Provider Page.
If there is medical necessity to request a Diagnostic Assessment (instead of a MH Evaluation) for a state-funded consumer, this must be pre-authorized. The one-page Request for Authorization form is used to request the Diagnostic Assessment.
Those who are endorsed for Diagnostic Assessment, please be aware of the following:
Medicaid law requires recipients to have the opportunity to select any willing and qualified provider of their choice (42 CFR 431.517) (Section 1902(a)(23) of the Social Security Act); therefore, providers must offer a choice of providers for Diagnostic Assessment and document such with a signed Provider Choice Form detailing the list of choices provided. This means that consumers cannot be required to use your agency and not be provided with a choice of providers for a Diagnostic Assessment.
AND
If you are an endorsed provider of Diagnostic Assessment, the Division indicates that Diagnostic Assessments cannot be completed exclusively to those consumers for whom you are doing Community Support Services. Please refer to your endorsement MOA.
Grant Information & Applications
Transition grants to assist adults with disabilities with certain costs associated with transitioning from facilities or institutions to community-based affordable housing are still available at a maximum of $1,000 per individual. The DHHS Housing Work Group has approved a policy change to consider households earning more than 30% AMI limits by area and household size.
As before, applications will be submitted via a sponsoring agency, which may be a DHHS office or nonprofit organization; therefore, we ask that you distribute this email to your regional or county offices, grantees, etc. to ensure the most widespread availability of this assistance.

presented at the Permanent Supportive Housing Coalition. Please use initials of your
consumer, not the full name.
The Permanent Supportive Housing Coalition meets the third Tuesday of every month at 1:30pm at Southeastern Center, 2023 South 17th Street.
Community Support workers can present their consumer(s) to representatives of the permanent supportive housing complexes in this area. The Coalition will make recommendations for your consumer. This meeting is also an excellent opportunity for networking and learning more about housing options in our area.
To present a consumer, please call Anita Oldham, Housing Specialist, at 910-313-6043 to let her know how many consumers you will be presenting, or for more information.
The State Mental Health budget this year includes funding for 400 units of Permanent Supportive Housing for disabled individuals. Any developer or organization interested in finding out how to access these funds to build affordable permanent supportive housing in our area, or just to get more information, please contact Anita Oldham, Housing Specialist, at 910-313-6043.
