|Take the Needs Assessment Survey|
|CoastalCare strives to ensure that quality behavioral health and intellectual/developmental disability services are available to the people of our community. Each year a Network Sufficiency Analysis of our State Funded network is conducted to determine if there are gaps in services or if there are underserved population groups that should be targeted for service development. |
In March of 2013, the annual Stakeholder Needs Assessment Survey was released. CoastalCare is now reissuing an improved version of our annual Stakeholder State Funded Network Needs Assessment Survey with the objective of increasing the response rate, improving respondents’ understanding of individual questions and collecting clearer results.
Individuals that receive services, their families, and all CoastalCare Stakeholders are invited to share feedback about behavioral health and intellectual/developmental disability services, using this on-line survey:
Stakeholder Survey: http://www.surveymonkey.com/s/7T7DD2X
Individuals/Family Members receiving services survey: https://www.surveymonkey.com/s/7MZT8DJ
Please complete the survey by May 22, 2013. Your experience and input is vital to the care offered in our community. The results of this survey will help design the fiscal year 2013-14 local benefit plan.Page 2 of 2.
